Rush Additional Pages Passport Service

This process is for adding pages to a valid passport.

If your scheduled departure is within 1 week then you are eligible for our Rush Additional Pages Passport Service.

In order to expedite your additional pages passport application, please follow each of the following steps. An incomplete or incorrectly completed application will delay the issue of your passport. To make the process easier to complete we suggest you print this page and follow the steps in the order presented.

Letter of Authorization

• Please print and complete two (2) copies of the Letter of Authorization.

Your Valid Passport

Evidence of Immediate Travel – Must provide a copy of one of the following:

• Copy of your round trip ticket indicating date and travel to a foreign country.
• Travel itinerary

DSP-4085 Form – You can obtain the form from either source. Fax or photocopies are not acceptable.

• Passport Acceptance Facility

Fees - Government and B & K Express fees are paid in separate checks:

• $60 government fee payable to US Department of State (please print the applicant’s date of birth on the memo line of the check), AND
• $149 B&K Express service fees Plus $92 shipping
